to me this looks like a helmsman's fault..he is supposed to be fixated on driving...instead he turned around, lost his concentration and drove more downwind..instead heading up when the spin started overpowering..put the crew into serious jeopardy..

0:13 that boom just missed his head, there to cool for life jackets. If that boom hit his head a life jacket would have been the only thing keeping him from going right to the botttom, he would have sadly been fish food. Always wear a life jacket because if a accudent happens it will save your life.

do try to sail up wind if the boat angles too far or catches weather helm? I dont really know what happened here?

it perfectly demonstrates the risk of a sudden helm followed by a chinese gybe when going with a spinnaker in butterfly mode.
in particular how fast such a thing can go wrong - with just fractions of a second to react. and in this case the crew didn't manage to prevent it. -

Sailing by the lee and too many distractions by having crew and "guests" aboard a racing yacht clearly "cruising" with possibly poor communications by a helmsperson and/or skipper = accidental gybe and "man overboard." Fairly good recovery and some good MOB techniques demonstrated: throw flotation in direction of MOB as soon as possible and order someone to clearly point as a spotter for the MOB so that the boat can attempt a recovery of the MOB but really less than ideal Hornblower stuff!
