All the crystal clarity guys are here:

This is why you keep all doors and hatches closed when you are under tow, those guys ditched too soon, we have a tow hook release on deck and in the wheelhouse, also a good idea to wear a knife.
-
its called girding, the hook should have been released far before it got that far,
-
some of the tugs i worked on had a lever in the wheelhouse that you could trip the tow hook with. in a emergency it released the tow rope, seen it done a few times to get out of trouble.
-
The tow was not cut as this would take lots of time even with a very sharp knife. the tow parted under the weight and the tug and the skipper should of never got into that position in the first place unless he had lost his engines or something else was wrong with the tug.
-
not man overboard but 4 men overboard.

This is a quite common thing that can happen to tugboats. When it is not straight in line with the load it pulls, the boat can drag itself under water side ways. In this case it's the river current that comes in from the side while the tugboat is held back by its towing line and a ship (?). The effect is the same. The crew was smart enough to cut the line with knifes. If tugboats are pulled under water it mostly happens while making sharp turns while tugging.
